Upload Path from field data

Request file upload instructions from file input By referring to the upload path from the data from the fields in that table.
such as
TestDocs table set upload folder = "files/Test/"

Fields TestDocsID, TesDocsFiles
Data 123, TES6509001-Docs-456.pdf

When adding this information, upload it to "files/Test/123/TES6509001-Docs-456.pdf"
