Custom template in Add Option Page

Hi,
I've noticed that when I add a custom template to 'Add Option Page', the form repeats itself with the first part empty. In this part, the form elements seem to be present but hidden, except that they take up space and there's a big white area at the top of the form. Is there a problem in the template or is it an error in my code? Has anyone been able to reproduce this malfunction?
NB: It works fine in Add/Copy and Edit pages

You need to post your code for discussion.
Right click the areas and select "Inspect" in your browser to check what HTML caused the space, then fix your Custom Template.
