noticed in the User Level Permissions that there is a duplicate table name entry
checked the menu/language text and can't see to see any duplicate entries
removed the entries from the table and re-migrated the security and the same entry re-appears
xml and lang files are set to regenerate
not sure where this is being retrieved from..
